As owner of ______________________, I give permission for
him/her to exercise and play in the fenced in area with other dogs. Kennel staff will exercise judgment in
socializing dogs. I do understand that this is a privilege for well behaved
dogs only. If any signs of aggression,
digging, destroying kennel property or jumping of the fence are observed, my
dog will not be allowed in the play yard but will be exercised in another
manner. I recognize and acknowledge that there are certain
risks of serious injury when dogs are allowed to socialize. I understand that
dogs can jump or dig under the perimeter fence to escape. I give permission to staff of Shady Oaks Boarding
Kennel to use first aid for minor injuries or veterinary services at owner expense.
I agree to hold harmless Shady Oaks
Boarding Kennel from injuries, lost collars, tags, toys.
